" there was a late change in the equipment being used to move pieces of track, which had inadequate lifting capacity for the planned works. ... Warnings were also ignored, with safety systems disabled to enable the work to continue. ...
Amey failed to take measures in order to adequately manage the risks presented when tandem lifting loads of the size and weight of those undertaken on 21 October 2018. ....... We are committed to safe working practices at all times
'cept on this occasion :LOL:
nobody reconized that the tracks were 30ft instead of 20 ft ,muppets

making the headlines all over :ROFLMAO:

"ORR also found that radio-based communication system between the operators and the lifting supervisor developed a defect. Despite replacement equipment being available on site, work continued with the inadequate communications equipment." :LOL:

" When lifting started, the on-board safety systems on both excavators showed that the weight being lifted was exceeding what they could safely handle. ORR found in its investigation that these warnings were ignored, and the safety systems were disabled to enable the work to continue. "

" ... had inadequate lifting capacity for the planned works. To overcome this, it was established that the length of the track to be lifted would need to be reduced from 30 to 20 feet. ...... led to one of the two excavators becoming overloaded beyond its capacity ........ when undertaking a tandem lift of a track panel later established to be 39ft long." :ROFLMAO::ROFLMAO:

you couldn't script it :rolleyes::(

didn't really know where to put this ....not so much an 'at work' matter, sort of, but certainly a health risk that HSE ain't involved in .......
the 'bad news' about these drinks has been out there for a long time, but folk seem hell bent on ignoring it .... they CAN kill you and cause all sorts of bodily damage and issues :(

Personally I don't like the stuff, so I don't really car either way, but there have been a ton of claims made about Red Bull (but not competing products with the same ingredients).
www.snopes.com/ is my first 'go to' site for a plausibility check and it seems they have plenty to say.

I'm not surprised, I think there will be a load of Muppets with lung problems in a few years caused from vaiping. Some people seam to think there are no consequences from over consumption of unnatural rubbish be it energy drinks, vaiping or sugar.
